Host: Diane in Corona, California recently sent us a home video about a very frustrating knee problem.
Diane: Hi doctors! I have had pain and swelling in my left knee for the past two years. I am very active and I show horses. What's wrong with my knee? Is there anything that you can recommend to help me with the pain and swelling in my left knee?
Host: Diane is here with us today also joining us is Orthopedic Surgeon Dr. Andrew Weiss. So welcome to both of you, thanks for being here. And doctor you examined Diane before she came out here, right?
Dr. Andrew Weiss: Right.
Host: Anything that you found in there?
Dr. Andrew Weiss: Yeah, she has a lot of fluid coming from with inside the knee and it's a problem that a lot of us have experienced at times in our life and it is a common problem that can be from many different sources. It can be from a trauma, it can be from an infection, it can be from inflammation that the body is just producing. And I think one of the most important things is trying to figure out where it may have come from and one of the ways to do that is to take the fluid out.
Host: And this causes you pain, prevents you from being as active as you like, right?
Diane: It burn, it is very painful.
Host: So would you be willing to let Dr. Weiss try to take some fluid off because it will be both what we call a therapeutic tap--
Dr. Andrew Weiss: And diagnostic.
Host: And diagnostic.
Host: So let's do it.
Dr. Andrew Weiss: Alright.
Host: And this is something that is done under sterile conditions and then you will take this fluid and send it off for analysis.
Dr. Andrew Weiss: Exactly and there is three different things that could come out. It could be clear, it can be clear yellow, it can be bloody and it can be even puss or green or infectious and we will know that pretty soon after putting the needle in.
Diane: Is it going to hurt?
Dr. Andrew Weiss: It is not going to hurt me at all and it will hurt you only a slightly. We have already numbed it up before and I think it should be okay, alright.
Diane: I am ready.
Host: And he is going right underneath the knee cap into the joint space.
Diane: It doesn't hurt that bad you guys.
Host: Did you hear that, it doesn't hurt.
Diane: It doesn't hurt.
Host: And nice, serous fluid which doesn't look infected or anything like that, no puss.
Diane: Are you going to be able to tell what's wrong from the fluid coming out?
Dr. Andrew Weiss: Yeah, basically I can look at this right now and I can tell you it is not consistent with any kind of an infection. It doesn't look like any type of crystal disease like Gout which can be caused from things where your body is unable to metabolize and it also doesn't look like any kind of trauma because it's just a little bit of what we call blood tinged and this is basically joint fluid that has a little bit of a blood, making it a little blood red. But what we are going to do now is we can send this off to the lab and we are going to have a lot of information once we get this back and this will tell us where it was coming from.
Diane: Can I ride my horse tomorrow?
Host: Now that you took the fluid off I want you to flex your knee for me if you can.
Diane: Oh My God!
Dr. Andrew Weiss: Does it feel, because I would like to get a really closed-in picture so I want to show her knees before and go ahead and let's look at the after. So potential for immediate release for this procedure, thank you so much for showing this Dr. Weiss.
