(Image source: Wikimedia Commons)   BY ELIZABETH HAGEDORN ANCHOR BRICE SANDER   An independent inquiry of the September 11 attacks in on the U.S. consulate in Benghazi cites "management deficiencies" and “systemic failures” by top State Department officials. The 39 page report released Tuesday night examined the deadly attack on the U.S. consulate in Libya which left U.S. Ambassador Chris Stevens and three other Americans dead. The report reads, " Systemic failures and leadership and management deficiencies at senior levels within two bureaus of the State Department resulted in a Special Mission security posture that was inadequate for Benghazi and grossly inadequate to deal with the attack that took place.” The review board did not single out any individual American officer for ignoring or violating his or her duties. The panel, however, took aim at the State Department for ignoring requests made from the U.S. embassy in Tripoli for more security. Fox News’s Catherine Herridge explains. “There was a steady stream of events leading up to the attack on 9/11 that showed security was deteriorating, but at the same time the State Department was increasingly relying on Libyan militia which proved to be inadequate on 9/11.” In a letter to Congress , Secretary of State Hillary Clinton, who’s at home recovering from a concussion, accepted all 29 of the report’s recommendations. Clinton has been out sick and will not be presenting the report in person at hearings later this week.” Retired U.S. ambassador Thomas Pickering and Admiral Mike Mullen will brief members of the House Foreign Affairs and Senate Foreign Relations Committees on Wednesday.
